Domino's is a major corporate sponsor of Operation Rescue.

Don't work or eat there.
I don't think it's asking for times, which would be more sensible. It's asking you for how many hours you might be available for. For instance I might be able to do the whole day, in which case I would say 6 in the top box and 9 in the bottom one. If you can only do an afternoon or morning I would put 4 in the top one and 5 in the bottom one.

It's up to you what you put, but your hours of employment should be agreed and documented in your contract before you start working there.

Good luck!I need help with filling out an application for a job at domino's pizza.......?
It is a pretty shite application form. This is your first test!!

They are looking for when you can start and when you can finish and how many hours you can do a day.

Depends on what you can do - if you are at school I guess 4 - 11 and then weekends 8 - 12 and see how it goes.

You can clarify it in the interview anyway and check when they want you.
Ok on the top line (from) put say 2 on the bottom line put 4

this reads on Monday you can work from 2-4.

Just follow that through for the rest of the week. Good luck with the application.I need help with filling out an application for a job at domino's pizza.......?
Can you go to the local one? Talk to the manager. Why settle just for Dominoe's, do some foot work, many little places are looking for help just like you.
Looking at the form and playing around with it, it seems likes you do it in 12 hr format.

eg.

Monday

from 8

til 11

Hope this helps
I would use a 12 hour format. Say you are available for work at 8am and can work until 10pm, I would just use "8a" to "10". If they're interested, they'll contact you which will give you the chance to explain further.

However, in the scenerio posed, having looked at the form, you could use a 24 hour format, such as "8" to "22". I see how that could work, but the employer might wonder what the hell "22" means because they are used to 12 hour formats (note the clocks).

All in all, I would go with a 12 hour format. At the interview, you can solidify your schedule. It's not a permanent thing on the application. It's more of an approximation so they can figure out if they need you during the hours that you're available.

Good luck with the job!
